Medical Forms

We are happy to assist you in the administration of medication and vitamins for your child in accordance with your doctor’s instructions. Since this is a service which the school is not legally obligated to perform, we ask that you fill out a form that lists dosage, time of dosage, and dates for which the medication or vitamins shall be given. These forms arebelow.  All medications must be in their original containers.

 

Students are not allowed to have any medication in their pockets, purses, backpacks, lockers, lunch bags, etc. All medication (including cough drops and lotions, but excluding chapstick) must be kept in the office.
